The Pennsylvania Departments of Aging, Health and Human Services Notification to facilities of an Incident that occurred on Saturday 10/26/2019 07:00 AM

Incident Summary:  A man entered a skilled nursing facility wearing a badge and carrying a binder (see attached photo).  He identified himself as a state employee and signed into the facility.  He then proceeded to enter a few residents’ rooms and interviewed them regarding the care and services being provided by the facility.  At some later time, the facility became concerned and notified the local police.  Police responded and are investigating the incident, as this man is not a state employee.  Thankfully, no residents or staff were harmed, and it does not appear that anything was stolen from the facility.  

The attached PDF documents include the various types of authorized badges state employees or state contractors display upon entrance to your facility.  The examples provided are from the Department of Aging.   If the individual indicates he or she is from the Department of Health or Human Services, the badge will clearly state the department. 

We are providing this information to all facilities so all appropriate staff are able to recognize Commonwealth authorized badges and the man involved.  Please follow your facility’s policy regarding the confirmation of who is entering your facility.  Ask for identification from anyone claiming to be a state employee.  If you have any concerns, please notify your supervisor and/or police. 

If you can identify the man in the attached photograph or have any questions, please notify your supervisor and the facility’s licensing entity immediately.
